Calvary Health provides concierge emergency medical services on a membership basis. In addition, we offer select regenerative medicine services, including but not limited to IV therapies, peptide protocols, and advanced wellness consultations. These services are not intended to replace your primary care physician or ongoing specialist care.

2. Membership Model

Our services are available exclusively to members. Membership does not guarantee unlimited access to services and may be subject to clinical appropriateness, scheduling availability, and geographic limitations.Membership tiers, included services, and pricing are outlined on our website and may change at any time with reasonable notice.Calvary reserves the right to decline membership applications or discontinue a membership for any reason, subject to a pro-rata refund if applicable.

3. Regenerative Services

Regenerative services are offered a la carte and are not included in base membership unless explicitly stated. These services may include advanced biologics, peptides, or off-label treatments. All treatments are provided only after physician review and informed consent.No regenerative therapy is guaranteed to produce specific outcomes.You acknowledge that many such treatments may be considered investigational or not FDA-approved for the specific use provided.

4. Medical Disclaimer

Calvary Health is not an insurance provider. Our services do not replace emergency departments in life-threatening situations. Always dial 911 or go to the nearest emergency room in such cases.

5. Privacy & Data

6. Payment & Refund PolicyAll services must be paid upfront or in accordance with your membership agreement. Missed appointments without 24 hours’ notice may be subject to a cancellation fee. Membership fees are non-refundable unless otherwise stated in writing.

7. Limitation of Liability

Calvary Health and its affiliates shall not be liable for any damages beyond the scope of direct clinical care provided, nor for third-party service failures, missed communications, or delays due to factors beyond our control.

We may update these Terms & Conditions from time to time. Continued use of our services constitutes acceptance of any changes.
